The high heel and bottom of sole were airbrushed black (no icing).  The pink on the high heel is a mixture of egg yolk, milk and food coloring that I painted on. The child's shoe is covered with traditional glaze (powdered sugar, corn syrup & milk) with royal icing brushed on for texture. The buckle is a molded cookie painted gold.  The baby shoe is covered with edible lace.

Fascinating. 

For the tops of the baby and child shoes, I baked the cookie dough over forms shaped out of foil.  The top/sides of the high heel is all one piece.  As soon as I took the cookie out of the oven and still pliable, I wrapped it around a boot form used to make gum paste boots.
