Thanks for posting this, Mallory! I was just about to post it myself. I hope people will consider Cookie Connection for the Learning Experience Award. (Hint, hint!) 
There's also a Cookie Award, which I encourage everyone to go for.
I just spent more then 20 minutes filling out the whole survey and when I pressed submission it said I my form fields were blank, which was not the case. Went to recheck it and wouldn't let me back into the survey. UMHHHHHH! Very frustrated.
Don't have the time to redo that again. I tried.
I wrote them a complaint too!

Bummer, I had no trouble with the form. They're really great there (especially Rosie), so hopefully they'll be able to fix things so you can vote again.
They only allow voting one time, everyone, so be sure you vote on every category that you want while you're in the form.
